3 risk factors to avoid that could delay dementia by over a decade

New research suggests that people who keep their blood pressure, blood sugar and smoking habits in check during midlife spend far longer living without dementia than those who do not.

The heart and the brain have always been linked, and doctors have long said that what helps one tends to help the other. A new study, published this week in Neurology Open Access, puts a number on that relationship. Researchers followed more than 12,400 middle aged adults for close to three decades and found that those without high blood pressure, diabetes or a smoking habit lived about 13 more years free of dementia than those who had all three.

Tracking heart health over three decades

At the start of the study, participants averaged 56 years old and showed no signs of dementia. Researchers checked each person for three vascular risk factors, hypertension, diabetes and current smoking, then tracked them for roughly 26 to 30 years through cognitive testing, phone interviews and medical records. People with none of the three risk factors remained dementia free for about 30 years on average. Those with all three made it just 17.5 to 18 years, depending on the analysis.

Each additional risk factor cost participants roughly four years of dementia free life, according to Josef Coresh, senior investigator on the study and founding director of the Optimal Aging Institute at NYU Langone. People with one risk factor averaged around 26 years without dementia, and those with two landed near 21 years.

The fairness gap inside the numbers

The pattern held across every group studied, but not evenly. Black participants consistently spent fewer years without dementia than white participants, and women outlasted men regardless of how many risk factors they carried. Researchers say that gap hints that early intervention could matter even more for some populations than others.

The study cannot prove that the three factors directly cause dementia, only that avoiding them tracks with more years of healthy cognition. Risk factors were also measured just once, at the start, so the data cannot show whether quitting smoking or treating high blood pressure later in life would shift someone’s trajectory.

How vascular damage may reach the brain

Researchers have a working theory for why this connection exists even though the study itself did not test it directly. High blood pressure, diabetes and smoking all damage blood vessels, and when that damage reaches the small vessels feeding the brain, tissue gets less oxygen and glucose while toxins clear out more slowly. The resulting inflammation may compound the problem, and some research suggests vascular damage can also accelerate the buildup of amyloid, the protein tied to Alzheimer’s disease.

What people can actually do about it

Experts point to the same basic playbook regardless of age. The American Heart Association’s guidelines call for a diet built around whole plant foods and roughly two and a half hours of moderate exercise a week. Heather Whitson, director of the Duke Center for the Study of Aging and Human Development, noted that a major clinical trial last year found that diet and exercise changes alone improved cognitive function in people already at risk, suggesting it is rarely too late to act.

Newer tools have also expanded the options. Text based quit smoking programs and nicotine replacement therapies have improved, and GLP1 medications used for diabetes and weight management show early promise for smoking cessation too. Doctors say the best next step for most people is simply raising the topic at their next appointment rather than waiting for a diagnosis to force the conversation.
