Ozempic babies, the double risk hiding in your pill pack

GLP 1, Ozempic, constipation

Ozempic babies are unexpected pregnancies reported by people taking popular weight loss and diabetes drugs such as Ozempic, Wegovy, Mounjaro and Zepbound. Specialists point to two main reasons they happen. Losing weight can restart regular ovulation in some women, and tirzepatide, the drug in Mounjaro and Zepbound, can make birth control pills less reliable.

For people taking tirzepatide, those two effects can hit at the same time. The body may be primed to ovulate again just as the pill is absorbed less effectively. That overlap is the part of the Ozempic babies story most people miss.

Which weight loss drugs affect birth control pills?

Tirzepatide is the main concern. It slows how fast food leaves the stomach, which can also slow how quickly pills are absorbed. According to the Food and Drug Administration prescribing information for Mounjaro and Zepbound, one dose reduced exposure to certain contraceptive hormones by about 20%. The labels recommend switching to a nonoral method or adding a barrier method, such as condoms, for four weeks after starting the drug and four weeks after every dose increase. Because doses usually rise step by step, that warning can apply several times.

Semaglutide, the drug in Ozempic and Wegovy, has not shown the same effect in studies. Those studies, however, measured patients already on a steady dose. Vomiting and diarrhea, which are common side effects of these drugs, can also interfere with how well any pill works. IUDs, implants, patches and vaginal rings do not depend on digestion.

Can weight loss shots make you more fertile?

For some people, yes, but indirectly. Polycystic ovary syndrome, or PCOS, often involves insulin resistance and high androgen levels that disrupt ovulation. Losing weight can improve insulin control and bring cycles back. In a 2017 trial of 176 women with PCOS and excess weight, nearly 44% of those on an older drug in this class became pregnant without fertility treatment, compared with about 19% on metformin. Specialists stress that these medications are not fertility treatments. Irregular ovulation can also come from thyroid problems, high prolactin, stress, overtraining, undereating or aging, so finding the cause still matters.

Do these drugs affect men’s fertility?

The research is still early. Findings presented at the Endocrine Society’s 2026 meeting suggested that testosterone and some sperm measures may improve in men with obesity, mostly those with low testosterone and mostly because of weight loss. One case report described sperm quality dropping on liraglutide and recovering after the drug was stopped. Doctors may look at both partners’ medications when a couple is trying to conceive.

When should you stop the shot before trying to get pregnant?

Ask your prescriber before you start trying. These drugs are generally not recommended during pregnancy because animal studies raised concerns about fetal development and pregnancy loss, and human data are limited. The Wegovy label advises stopping at least two months before a planned pregnancy because semaglutide stays in the body for weeks.

Tirzepatide labels do not set the same timeline, so guidance varies by provider. Anyone who becomes pregnant while taking one of these drugs should contact their prescriber and begin prenatal care. Early research has not found a clear rise in major birth defects after accidental exposure, but that does not prove the drugs are safe in pregnancy. People with diabetes will also need a plan to manage blood sugar after stopping.

Why does this matter so much for Black women?

The stakes of an unplanned pregnancy are not equal. CDC data have long shown that Black women in the United States die from pregnancy related causes at about three times the rate of white women. For Black women using these drugs, planning ahead for contraception and pregnancy timing is a matter of safety.

What is happening outside the United States?

Regulators are watching. In the United Kingdom, the Medicines and Healthcare products Regulatory Agency has received dozens of pregnancy reports linked to these drugs and has urged users to use effective contraception, with a specific warning for Mounjaro users who rely on the pill.
