Getting a clean bill of health from routine labs can feel reassuring, but a large new study suggests that reassurance may be incomplete. Research tracking more than 335,000 people over nearly 14 years found that two overlooked, largely silent markers can combine to raise the risk of heart attack and stroke in ways that standard checkups frequently fail to catch. Notably, the people most at risk of being blindsided are often those who appear perfectly healthy based on typical screening results.

Inside the research

The study drew on data from the UK Biobank, a large, long running research project following adults who had no cardiovascular disease at the time they enrolled. Researchers set out to examine two markers that had each been individually linked to heart risk in prior research: the Triglyceride Glucose index, a measure of insulin resistance calculated from triglyceride and glucose levels already included in a standard blood panel, and vitamin D levels.

While each factor had been studied on its own, researchers had not previously examined what happens when both are abnormal simultaneously. That gap is notable given how common and quietly symptomless both conditions tend to be, and how rarely doctors evaluate them together. Vitamin D deficiency, researchers noted, extends well beyond bone health, also influencing blood vessel function, inflammation and blood pressure regulation.

When both numbers are off, risk climbs sharply

People with both a high Triglyceride Glucose index and low vitamin D levels were roughly 26% more likely to experience a major cardiovascular event, including heart attack, stroke, sudden cardiac arrest or cardiovascular death, compared with people who had neither issue.

What stood out most to researchers wasn’t just that risk increased, but how it increased. Rather than the two factors simply adding together, as might be expected, the combination appeared to amplify overall risk beyond what a simple addition would predict. That kind of interaction, where the combined effect outweighs the sum of its parts, is what researchers found particularly significant.

The effect was most pronounced among people without diabetes or obesity, especially those with a body mass index under 30, precisely the group least likely to be flagged as high risk during a routine visit.

How to check these numbers yourself

Both markers are measurable through tests most patients already have access to, and both are considered addressable once identified. A standard blood panel already includes triglycerides and glucose, the two values needed to calculate a Triglyceride Glucose index. Adding a vitamin D test to that same blood draw provides a fuller picture in a single appointment.

According to the study, a Triglyceride Glucose index at or above 9.4, paired with a vitamin D level below the sufficient threshold, represents the highest risk combination identified. A doctor can help interpret where individual results fall and whether further evaluation is warranted.

Addressing both factors, not just one

Researchers emphasized that tackling both issues together, rather than focusing on just one, may offer stronger protection. For low vitamin D, straightforward interventions include more sun exposure when possible, incorporating foods like fatty fish and egg yolks, and supplementation when needed, particularly during winter months with limited sunlight.

Improving insulin resistance generally comes down to lifestyle changes, including regular strength training, reducing consumption of ultra processed foods, prioritizing quality sleep and managing stress. Researchers noted that addressing both markers in tandem may provide more meaningful protection than improving just one in isolation.

Why a clean bill of health isn’t the full picture

Perhaps the study’s most important takeaway is that cardiovascular risk doesn’t require a formal diagnosis to be present. Adults without any known health conditions but with numbers trending toward this high risk combination still face elevated risk, and catching that pattern early is precisely the point of testing for it.

Two simple values that are rarely checked together may paint a more complete picture of cardiovascular health than either number does on its own.
